I am requesting the City of Charlotte to deliver wood chips to property I own at the following address:

You are agreeing to the following. Please read carefully.

• I understand that this may be 13 cubic yards of wood chips.
• Once it is dumped, the City will not be responsible for any excess I do not use.
• I will take responsibility to comply with all City Ordinances.
• I understand that the wood chips will be dumped at the end of my driveway unless I request otherwise (attach a drawing of where you would like the wood chips dumped).
• The City will not dump wood chips in the street where traffic or sidewalks will be obstructed.
• I understand the City will make every effort to deliver a good quality load of wood chips.
• Once the load is dumped, the City will not be responsible for trash, rakings or other debris that was part of the wood chips, which includes the normal decomposition of the wood chips.
• I understand the City will not re-load the wood chips once it has been dumped.
• In consideration of this service, I agree to indemnify and hold harmless the City of Charlotte from and against all damage, including injury to persons or damages to property, expense or other liability which may result from, arise out of, or be brought by the depositing of wood chips on my property. 
• Once this form is signed and submitted, your name will be placed on a list on a first-come, first-served basis.
