Please fill out and submit this form if you will be attending the Charlotte-Mecklenburg All Hazards Advisory Committee (AHAC) Conference. 

To Register:

1. Fill out the form below with your information. Note the registration number you receive once you have submitted the form.
2. Pay the registration fee of $65 at http://charlottenc.gov/Fire/CFDIntheCommunity/SiteAssets/ConferencePayment.html.
   1. On the first page, you will be asked for the conference name. Please enter “AHAC Conference”.
   2. You will also be asked for a registration number. Please enter the registration number you receive after filling out this registration form.
   3. The next page will ask you to enter the conference registration fee amount. Please enter $65.00 and click submit.
   4. Payment information is entered on the final page.
   5. You will receive a digital receipt upon payment.
